The Dietician supports the primary care physician network by providing nutritional assessment, education and management to patients with chronic conditions who are at risk for disease progression, complications, cognitive and functional decline. In collaboration with the physician and interdisciplinary team, the Dietician develops and implements an individualized care plan designed to promote the patient’s understanding and management of their condition, optimize quality of life, and improve clinical outcomes. This role will also develop various nutritional programs (adult and paediatric focused) for the provider organization.
RESPONSIBILITIES & OBJECTIVES
1. Support provision of PSS & MHPSS service within the community based rehabilitation project:
• Assess psychosocial needs of all beneficiaries coming to the project activities (men, women) using specific tools
• Design a tailored action plan to respond to the psychosocial needs of each person (when needed)
• Organize and facilitate group sessions for beneficiaries when needed.
• Ensure the implementation of a quality PSS intervention for beneficiaries showing psychological distress.
• Establish of long-term and short-term objectives that ensure safety and stability and enhance the flexibility of the individual and the family to bring their lives back to normal
• With the consent of the beneficiaries, implement individual intervention plans that ensure safety and stability and enhance the flexibility of the individual to bring his/her life back to normal
• Have a good mapping of services
• Refer beneficiaries to specialized services when needed and follow up
• Use various communication means (written, spoken, drawn, etc.) during activities to ensure effective inclusion
2. Support and contribute to effective implementation of project objectives;
• Participate in the trainings as and when requested
• Document any protection concerns or any incident/ act that causes harm to beneficiaries and report them promptly to the Line Manager and Social worker team leader.
• Respect principles of data protection and security, and all guidelines of documentation within the project and keep data records
• Maintain complete individual files for each person visited, including detailed descriptions of services provided
