About Project
According to the new community based rehabilitation CBR program and in collaboration with LHF/ UN OCHA, Rahma hospital- Association Des Jeunes Islamiques (ADJI) will provide community based multidisciplinary rehabilitation services to CWDS, PWDS & elderly to meet their emergency needs in order to prevent long-term functional/physical impairment and ensure adequate referral to specialized services.
The project will cover the most vulnerable areas of Mhammara, Ouadi Khaled (Amayer), Bebnine, Halba, Mazareaa Jabal Akroum, Berqayel, Machta Hamoud in Akkar district and in Trablous El-Qobbe, Trablous El-Tell, Trablous El-Haddadine, El-Hadid, El-Mharta, Trablous et Tabbaneh, Trablous Ez-Zeitoun, Mina N3 in Tripoli district.
In Tripoli, ADJI will deliver the services through its rehabilitation center and mobile team, while in Akkar, ADJI will deliver the services in 9 locals rehabilitation center/clinics through contracted therapists distributed over the areas of intervention to transfer its knowledge to local actors, create a sustainable action and improve accessibility to the services by release the transportation burden.
The Community-Based Rehabilitation Team Leader is responsible for overseeing and coordinating the activities of a rehabilitation team focused on providing services and support to individuals with disabilities within the community. This role involves leadership, program development, collaboration with various stakeholders, and ensuring the delivery of effective rehabilitation services.
Key Responsibilities:
Team Leadership and Supervision:
Provide leadership and guidance to a multidisciplinary team of rehabilitation professionals, including therapists, Speech therapist, Occupational therapist.
Delegate tasks, set performance expectations, and conduct regular performance evaluations.
Foster a collaborative and supportive team environment to maximize efficiency and effectiveness.
Program Development and Implementation:
Collaborate with stakeholders to design and develop community-based rehabilitation programs that address the diverse needs of individuals with disabilities.
Create individualized rehabilitation plans for beneficiary, considering their goals, abilities, and specific challenges.
Monitor and evaluate program outcomes, making adjustments as necessary to ensure effectiveness.
Beneficiary Assessment and Support:
Conduct comprehensive assessments of clients' physical, cognitive, emotional, and social needs to determine appropriate rehabilitation interventions.
Develop and implement beneficiary-centered intervention plans to enhance their independence, functional abilities, and overall quality of life.
Provide counseling and emotional support to beneficiary and their care givers.
Stakeholder Engagement:
Establish and maintain collaborative relationships with local healthcare providers, social service agencies, educational institutions, and other relevant organizations.
Actively participate in community outreach and awareness activities to promote the rehabilitation services offered by the organization.
Documentation and Reporting:
Maintain accurate and up-to-date records of patient assessments, progress, and intervention plans.
Prepare regular reports on team activities, program outcomes, and client success stories for internal and external stakeholders.
Quality Assurance and Compliance:
Ensure that all rehabilitation services adhere to ethical standards, best practices, and relevant regulations.
Monitor the team's compliance with documentation, confidentiality, and safety protocols.
Qualifications:
Bachelor's or Master's degree in Physical therapy or a related field.
Proven experience in rehabilitation services, with a focus on community-based support and intervention.
Strong leadership and team management skills.
Excellent communication and interpersonal abilities.
Knowledge of disability-related issues, resources, and support systems.
