Responsibility
As a member of the Hospice Care Team, she/he will work under the direction and the supervision of the Medical Director regarding all medical issues. She/he will also be guided and directed by the Nursing Director, in providing skilled nursing assessment, planning and care in order to maximize the comfort and health of patients and families.
The Hospice Nurse is responsible for identifying patient/ family needs and for providing supportive care in accordance with the plan of care set with the Medical Director and the Nursing Director.
Functions
Under the guidance and supervision of the Medical Director and the Head of the Nursing Unit, the Hospice Nurse shall carry out the following:
Executive tasks:
– Perform evaluation of patients for eligibility for SANAD services.
– Develop a care plan and advanced directives with patients and their families on admission.
– Review care plan with Medical Director and the Nursing Director on a regular basis.
– Provide nursing services to SANAD patients in their homes under the supervision of the Medical Director and the Nursing Director.
– Assess the ability of family members to provide care for the patient.
– Facilitate agreement of family members regarding care plan.
– Obtain and analyze data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data and its findings in care plan.
– Draw a plan for training and role designation for family members.
– Coordinate care with the primary physicians of patients when needed.
– Report on all organizational, medical, nursing and technical care matters to Medical Director and the Nursing Director .
– Uphold the mission and goals of SANAD.
Organizational tasks:
– Ensure proper documentation of planning, coordination and delivery of patient/family care.
– Provide for the regular interdisciplinary team meetings the development, review and revision of the patient / family plan of care, including exchange of information, problems or unique issues and professional support provided.
– Maintain medical records (including equipment and medications), assuring accuracy, completeness and compliance with registration regulations, legal and ethical standards.
– Participate in efforts to increase community awareness on end-of-life issues.
Qualifications
– Registered Nurse with active license and registered in the Order of Nurses.
– At least 3 to 5 years of a valid work experience as a professional nurse
– Skills in administrative performance, which includes expertise with communication, both written and oral, documentation requirements, service coordination.
– Excellent communication skills, empathy, and compassion. Open minded, nonjudgmental, respectful of others and their beliefs.
– Flexible, proactive, problem solver, trustworthy, and willing to accommodate to difficult and unexpected situations.
– Fluent in Arabic and English and/or French.
– Knowledge of basic computer software such as Word and Excel.
– Ready to travel to receive additional training on support of terminal patients and their families.
