Summary of the key duties performed by case management workers:

Assessment: Case management workers conduct comprehensive assessments to evaluate Beneficiaries' physical, emotional, social, and financial needs. They gather information through interviews, observations, and collaboration with other professionals to identify the most pressing issues and develop appropriate intervention plans.

Care Planning: Based on their assessments, case management workers create individualized care plans that outline specific goals, objectives, and strategies to address Beneficiaries' needs. They collaborate with Beneficiaries, their families, and relevant service providers to establish realistic and achievable plans.

Service Coordination: Case management workers act as the central point of contact and coordinate services on behalf of their Beneficiaries. They liaise with various professionals and organizations, such as healthcare providers, social workers, housing agencies, and legal services, to ensure Beneficiaries receive the necessary support and resources.

Referrals and Linkages: Case management workers identify and refer Beneficiaries to appropriate community resources, such as counseling services, financial assistance programs, vocational training, or healthcare facilities. They establish and maintain relationships with these resources to ensure timely access and continuity of care.

Advocacy: Case management workers advocate for their Beneficiaries' rights and needs within the healthcare, social services, and legal systems. They may accompany Beneficiaries to appointments, hearings, or meetings to provide support, clarify information, and ensure that Beneficiaries' voices are heard.

Monitoring and Evaluation: Case management workers regularly monitor Beneficiaries' progress and reassess their needs to determine the effectiveness of interventions. They document and maintain accurate records of services provided, outcomes achieved, and any changes in Beneficiaries' circumstances.

Crisis Intervention: In situations of crisis or emergencies, case management workers provide immediate support and connect Beneficiaries with appropriate emergency services, ensuring their safety and well-being.

Education and Guidance: Case management workers educate Beneficiaries and their families about available resources, rights, and entitlements. They provide guidance on navigating complex systems, making informed decisions, and developing skills for self-advocacy and self-sufficiency.
