JOB SUMMARY: Responsible for coordination of care pathways following a case management Model for people with disability (PwDs) who have complex care needs.

MAIN TASKS AND RESPONSIBILITIES
In accordance with established project guidelines, this position will be the focal point in ensuring that target population receives intended services in the assigned area:
• Multidisciplinary Case Management: Serve as the main focal point for coordination, monitoring, and reporting referrals and service provision within his/her area for the case management.
o Will implement and monitor the case management plan of identified cases according to set criteria, for moving people with disability (PwD) through the process of change, and toward the goal of self-sufficiency and achieving their highest level of functioning
• Provision of Assistive Devices: Acts as focal point, in coordination with IMC medical advisors, Inclusion team, and Primary health center staff for approval, coordination, monitoring, and reporting of provision of assistive devices at Primary health care centers or externally contracted Rehabilitation Centers
• Provision of Rehabilitation Therapy: Acts as focal point, in coordination with IMC medical advisors, Inclusion team, and Primary health center staff for approval, coordination, monitoring, and reporting of provision rehabilitation therapy (physiotherapy, occupational, speech therapy) at Primary health care centers or externally contracted Rehabilitation Centers
• Conduct monitoring visits to Primary health care centers and contracted referral centers
• Monitor service delivery quality in contracted referral centers (diagnostics/rehabilitation centers) and reports challenges.
• Follow up on data entry inside Primary health care centers and monitor quality and accuracy of data entered and provide support for data entry staff.
• Monitor and report any challenges and/or gaps identified to inform adjustments to the program
• Complete project documentation for assigned activities. Assist with identifying information for case studies and reports on promising practices.
• Participate in meetings with health field management and community health staff to track indicators and prepare weekly reports on people with disability (PwD) project

JOB REQUIREMENTS *
MINIMUM QUALIFICATIONS
• Typically, a Bachelor’s in Social Work or Social Science. Equivalent combination of relevant education and experience may be substituted as appropriate.
• Typically, 3+ years of relevant experience in project support, preferably related to disability.
• Good command of written and spoken Arabic and English
• Observation, active listening and analysis skills with ability to make sound judgment
• Good relationship management skills and the ability to work closely with local partners and community members
• Attention to detail, accuracy and timeliness in executing assigned responsibilities
• Proactive, results-oriented and service-oriented
Desirable
• NGO experience
• Experience monitoring projects and collecting relevant data preferred
• Able to take initiative within a defined framework
• Microsoft office skills
