– **إدارة رعاية المرضى:**

– **تنسيق الإحالات:** تسهيل وإدارة الإحالات إلى المستشفيات، وضمان تقديم الرعاية في الوقت المناسب وبشكل مستنير. متابعة تقدم المرضى، وضمان الالتزام بخطط العلاج، وقبولهم في الوقت المناسب.
– **تنسيق رعاية الأطفال:** متابعة قبول وعلاج الأطفال دون سن 18 سنة، وضمان تلقيهم التدخلات اللازمة.

– **التنسيق مع مراكز الرعاية الصحية الأولية (PHCCs):**
– **جدولة المواعيد:** التعاون مع مراكز الرعاية الصحية الأولية وفريق التغذية في RI لترتيب زيارات الرعاية بعد الولادة للأمهات الجدد.
– **استمرارية الرعاية:** ضمان تلقي الأمهات الجدد للرعاية والمتابعة اللازمة.

**Position Overview:**

As a nurse with Relief International (RI), you will be pivotal in overseeing the care process for pregnant women and managing complex care coordination. You will operate under the guidance of the public health officer to ensure that high-risk pregnancies receive appropriate medical attention and support.

**Key Responsibilities:**

– **Patient Care Management:**
– **High-Risk Pregnancies:** Assess and manage referrals of pregnant women with conditions such as preeclampsia, gestational diabetes, or multiple gestations. Coordinate with RI’s field medical coordinator for hospital admissions.
– **Referral Coordination:** Facilitate and manage referrals to hospitals, ensuring timely and informed care. Track the progress of patients, ensuring adherence to treatment plans and prompt admissions.
– **Childcare Coordination:** Oversee the admission and treatment of children under 18, ensuring they receive necessary interventions.

– **Coordination with Primary Health Care Centers (PHCCs):**
– **Appointment Scheduling:** Work with PHCCs and RI’s nutrition team to arrange timely post-natal care visits for new mothers.
– **Continuity of Care:** Ensure that new mothers receive necessary follow-up care and support.
– **Education and Resources:** Identify and address training needs for hospital staff, collaborating with the public health officer to develop and implement relevant training.

– **Documentation and Compliance:**
– **Comprehensive Documentation:** Ensure complete and accurate documentation of patient interactions and care, including medical histories and referral details.
