الوصف الوظيفي:
- استلام إحالات تتعلق بالأفراد الضعفاء الذين يواجهون صعوبات في الوصول إلى خدمات الرعاية الصحية، وتوفير أفضل الفرص لهم للتواصل مع الخدمات الصحية في الوقت المناسب.
- تقديم المساعدة المطلوبة للمستفيدين المستهدفين وإجراء الإحالات اللازمة وفي الوقت المناسب إلى مراكز الرعاية الصحية الأولية والخدمات الصحية المتخصصة (مثل الإحالات إلى المستشفيات بما في ذلك القبول النفسي، القبول بسبب سوء التغذية الحاد، الرعاية الطارئة للأمومة، وغيرها) وإحالات إلى خدمات المساعدة الأخرى غير الصحية عند الحاجة، مع احترام خصوصية وحقوق المستفيدين.
- متابعة الحالات المحالة لضمان وصولهم إلى الخدمات اللازمة.
- تقديم جلسات توعية غير رسمية للمستفيدين المحالين حول عدة مواضيع إذا لزم الأمر، مثل الأمراض غير السارية، التغذية في مرحلة الطفولة، والأمراض المنقولة عن طريق المياه والنظافة.
- متابعة القضايا البيئية المستلمة وضمان الجهود المنسقة بين وزارة الصحة العامة وموارد المياه والصرف الصحي والمجتمع الصحي.
- ضمان تقديم تقارير دقيقة عن الأنشطة إلى المشرف باستخدام نظام RAIS للحالات الفردية، وتعبئة ورقة إكسل لتقارير متابعة أحداث الصحة البيئية.
- أداء مهام أخرى حسب الحاجة أو تكليفها.

#### Job Description:
– Receive referrals related to vulnerable individuals facing challenges in accessing healthcare services and provide them with optimal opportunities to connect with timely health services at health facilities.
– Provide necessary assistance to targeted beneficiaries and make timely referrals to primary healthcare centers and specialized health services (e.g., referrals to hospitals, including psychiatric admissions, admissions due to severe malnutrition, emergency obstetric care, and others) as well as referrals to non-health-related assistance when needed, ensuring confidentiality and respect for the beneficiary's rights.
– Follow up on referred cases to ensure access to required services.
– Conduct informal one-on-one awareness sessions for referred beneficiaries on various topics if needed, including but not limited to non-communicable diseases, infant and young child feeding (IYCF), and waterborne diseases and hygiene.
– Address environmental issues received and ensure coordinated joint efforts/action plans between the Ministry of Public Health's Emergency Services Unit, WASH, and health actors in the areas of responsibility.
– Ensure timely reporting of activities to the supervisor using the RAIS system for individual cases and filling out the Excel sheet for reporting/follow-up of environmental health events.
– Perform other duties as required or assigned.
